becoming a path4ems service provider
The requirements for being designated as a provider of Path4EMS program services are established in accordance with CRS 25-3.5-208 and include the education, evaluation, and treatment of Colorado’s EMS Professionals. Program service providers are compensated under a purchase order type process, which is simple and flexible. Additional details are included below.

Statement of Work
The Statement of Work or SOW, is the guiding document for Path4EMS program services. It includes a program description and pertinent definitions, along with specific expectations and reimbursement amounts. A copy of the current SOW can be found here.
Required Documentation
Along with the SOW, a number of items are necessary for us the very your business information and establish your payment account. These are:
a Colorado Secretary of State ID number
a SAM.gov ID number
Form W-9
ACH/EFT Form for electronic deposits
voided check
proof of General Liability Insurance
The Path4EMS Program Coordinator can assist in obtaining or completing any of these items
Reporting & Invoicing
A monthly report is used to collect necessary program information. It includes the amount of time to be reimbursed for each participant, along with their current status. At the end of a participants treatment course, we will also ask that you provide them with a link to an anonymous survey to help collect general program data.
